You can help us to help you

Please let us know if you change your name, address or telephone number and do everything you can to keep appointments and tell us as soon as possible if you cannot.

Please ask for home visits by the doctor only when the patient is housebound.

Remember, you are responsible for your own health and the health of your children. We will give you all the professional help and advice we can – please act on it!

TELEPHONE SYSTEM UPDATE

We have worked on the problems with our telephone system and are delighted to inform you of the improvements. Our telephone improvements for June 2015 document highlights the change in answered calls. We are confident that we now have the structure in place to manage the calls efficiently. We hope to have even better results in the coming months!

Referral Booking Management Service transfer to CSC on 1 July 2015.

Patient Notice for RBMS Transfer

From 1st April 2015, all patients registered at any GP practice are required to be allocated a named accountable GP.

What does ‘accountable’ mean?
The named accountable GP is responsible for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each patient as required. However, this does not mean that they will be the only GP or clinician who will provide care to that patient.

 

HAVING TROUBLE GETTING AN APPOINTMENT?

We still see a large number of patients for appointments that could have been dealt with by a Pharmacist or NHS Direct. If your appointment is for a minor ailment, please see your Pharmacist or call NHS Direct on 0845 4647 for advice before taking up an appointment with your GP.

 

Please Note: We have two new community services based at Firsway.

BLUESCI offer a simple stepping stone approach to help you improve and maintain your wellbeing. For further information contact 0161 9122177 or ask your GP.

Trafford Carers now offer a walk in advisory service for all carers every Monday 9am to 4.30pm. The helpline is 848 2400.

Patients who are new to the Practice will now be invited to see our Health Care Assistant for a New Patient Check.
